2023 Health and Happiness Survey
This survey is a collaborative project between Lake County Build a Generation and St Vincent Health. We are collecting information to better understand the factors that are impacting our community health so we can work together to make Lake County a healthier place for all of us to live. The survey should take less than 15 minutes to take. Choose the English or Spanish version after you click on the link below.
The Health and Happiness Survey is an essential part of St. Vincent Health’s Community Health Needs Assessment. We perform this assessment every 3 years. You’ll find previous years’ reports linked in the footer of this website, right column.
The survey is completely anonymous and voluntary. You can skip any questions you are not comfortable answering and can stop taking the survey at any time. Your answers will be combined with answers from other Lake County community members to help describe the well-being of Lake County as a whole.
If you would like to provide your email address to get updates on the project, there is a separate link at the end of the survey to do that. Signing up will not impact the confidentiality of your responses. There is no way to link the two responses together.
